WHY THIS FORM IS REQUIRED

To provide a seamless VIP Travel Planning & Booking experience, Zensseys may assist with securing travel reservations on your behalf. Before any reservations can be made, we must receive your authorization to use a payment method for approved travel purchases.

For your security, Zensseys does not request credit card numbers through online forms. Payment information will be collected directly from the cardholder by phone and entered into secure, PCI-compliant booking systems utilized by our travel agency partners and suppliers.

This form authorizes Zensseys to coordinate and process approved travel bookings on your behalf using the payment information you provide separately.

BOOKING AUTHORIZATION

I authorize Zensseys to use payment information that I provide directly by phone to secure travel reservations and travel-related services that I have approved.

I understand that no reservations requiring payment will be completed without my approval.

I understand that travel suppliers may require deposits, partial payments, or full payment at the time of booking.

ACKNOWLEDGEMENTS

I understand that:

• Travel reservations are subject to supplier availability and pricing at the time of booking.

• Prices may change until reservations are confirmed.

• Airlines, hotels, resorts, cruise lines, tour operators, and other travel suppliers maintain their own cancellation, refund, and change policies.

• I am responsible for reviewing all confirmations and notifying Zensseys immediately of any errors.

• Zensseys acts as a travel advisor and booking intermediary and is not the merchant processing travel transactions.
